Supervision Agreement Changes

​Effective September 2018, the State Medical Board of Ohio no longer requires Physician Assistant Supervision Agreements to be filed with or approved by the Board. Supervising physicians are responsible for keeping the completed and signed agreement with the physicians records and it must be readily accessible for on-site inspection.

Since supervision agreements are no longer filed with the Board, there is no expiration nor renewal of them. They are living documents that remain active until terminated. They can be amended at any time to change the scope of practice or PA responsibilities. For your convenience, you may use the forms below as your template or create your own documents.
