Q1: How do I change/update my mailing address?
A:
In order to submit an address change, please do the following:
Using a
GOOGLE CHROME browser,
visit our eLicense website at:

Click “
Log In/ Create Your Account
”
Under “
Existing Users
”, enter you email and password, then click “
Login
”
You may need to request a password reset to get started.
This link will be emailed from a No Reply account and may reach your Junk or Spam Folder.
If you do not recall/need to change your email address on file, please contact us at
license@med.ohio.gov
for assistance.
Under “
Your Licenses
”, click “
Options
” on your license(s)
Within the drop-down menu, click “
Change Address
”
Click the “
+Add Address”
button
Enter the new complete address
Privacy option
: If you wish to keep your public address private, enter “Private” within the address box for your public address only.
You must provide a full address for
Mailing Address
.
Click
Save
A Geo Validated Address option window will provide the official USPS address.
Move mouse over street address option until hyperlink appears
Once hyperlink appears select the address by clicking on it.
Click
Done
.
If you want to add another address, click the “
+Add Address”
button and repeat.
Under
Mailing Address
option, click the down arrow and select the address on file you want for the mailing address.
Under
Public Address
option, click the down arrow and select the address on file you want for the public address.
Click
Submit
to update addresses on file.
*NOTE: You cannot delete any addresses on file.
Q2: How do I retrieve my login information or update my email?

with your license number and current email to request this information.
Q3: I need to renew my license.

Pursuant to Ohio law, twenty dollars of each renewal fee goes to the Physician Loan Repayment Fund which provides loan repayment assistance to physicians who commit to practice in underserved areas. Further information regarding the Ohio Physician Loan Repayment Program can be found on the Ohio Department of Health’s website.

Note:

Q5: How do I get confirmation of my payment?
A:
Payment confirmations will be sent to the email address on file after the payment has been submitted. If you need another copy, you may obtain one from the dashboard under My History.
Q6: Where may I find more information about a specific physician, such as his or her education, training and/or practice information?
A:
You can look up a physician by going to
https://elicense.ohio.gov
and entering the physician's first and last name before clicking Search.
Q7: When I submitted my renewal online, why did I not receive a new wallet card?
A:
The board discontinued issuing wallet cards on September 29, 2015. You will no longer receive a wallet card at the time of initial licensure or renewal, pursuant to HB 64. The confirmation of your renewal will be emailed to you upon completion of the online renewal process. A duplicate if available on the dashboard under Options.

Q10: How do I submit a Mid-Year Program change for a Training Certificate?
A:
If you change programs, complete the
Training Program Change
form and have the new program email it directly to the Medical Board at
license@med.ohio.gov

Q11: What is the loan repayment fee on the physician license renewal
?
A:
Pursuant to Ohio law, twenty dollars of each renewal fee goes to the Physician Loan Repayment Fund which provides loan repayment assistance to physicians who commit to practice in underserved areas. This is not a new fee and the total renewal fee paid by physicians has not changed. Q12: How do I get a license verification form completed and sent to another state?
A:
OFFICIAL VERIFICATION REQUEST (also called a letter of good standing)
- We no longer complete paper verification forms from other state medical boards. All other medical boards, Canadian provinces and medical councils are aware of our policy and accept Ohio electronic verification letter. We will include the state exam results where applicable with each verification. Prior to July 1, 1999, training certificates were not mandatory in order to engage in an accredited training program in Ohio. Therefore, the board cannot verify the training certificates issued prior to July 1, 1999. The NBME, NBOME, NBPME and the USMLE scores must be obtained directly from those entities.
Steps to Request an Official Verification:
Using Google Chrome, go to

Select
LOGIN/CREATE YOUR ACCOUNT
If you have already registered, enter your email and password under the
Existing Users
Login
If you have not registered, select
I HAVE A LICENSE
and complete the site registration process
NOTE:
If your information may not be current on file, please email your license number and a current email to Licensee Services at
license@med.ohio.gov
before beginning the registration process.
After you are logged in, find the license(s) on your dashboard
Click on the
Options
button on that license tile
Select
SEND VERIFICATION
Complete the Submit Verification to Another Entity form
Repeat the above steps if you have more than one license to be verified
Pay the fee(s) online through your shopping cart with Visa, Mastercard or Discover. The $50 verification fee is applicable per license for: MDs, DOs, DPMs, Massage Therapists, Cosmetic Therapists, Respiratory Care Professionals, Dietitians, Training Certificate holders and Special Certificate holders.
