PODIATRY CHECKLIST

Please read and follow the instructions carefully. If you have specific questions after you have read all the requirements, please call the Medical Board offices at (614)466-3934 and follow the prompts to the Licensure Department.

1. Fill out the "Request for Application and Preliminary Education" Form. (click here to download this form) 

When this form is completed, mail the original form to the  State Medical Board of Ohio, 77 S. High St., 17th Floor, Columbus, Ohio 43215-6127.  Please do not submit any documents (diplomas, CVs, etc.) or fees with this form.  The full application for licensure will be mailed to you within a week of our receipt of this properly completed form.  

2.    As indicated in Step 1, above, you will receive a full application for licensure in the mail. Complete that application and submit it , along with the appropriate fees, to the State Medical Board of Ohio. Forward any other forms (recommendation forms, state verifications, etc) that were checked on your application instructions to the appropriate agency.

If you are applying by endorsement:

 Processing ordinarily takes 10-12 weeks from our receipt of your application.    If your application file is incomplete at the time it is reviewed by the licensure staff, you will receive an incomplete letter from the Board outlining the missing information or forms needed to complete your application.

If you are applying for the PMLexis examination:

The PMLexis is given twice a year in June and December, on a nationwide basis.  Failure to file the application you receive from the the Medical Board will result in ineligibility for the examination,so check the deadlines carefully.  The deadline for submitting an application is ninety (90) days prior to the examination, unless otherwise noted.  Results of the exam are released approximately sixty (60) days after it has been administered. Applicants who pass the examination will be mailed a "Certificate Issuance" application to apply for an Ohio license.  Applicants who do not pass the examination will be mailed an application to reapply for the next examination.  Passing the examination alone does not grant you a license to practice podiatry in Ohio.  You must complete the application and send it to the Board with the appropriate fees.

3.  Check the status of your application.

 If you wish to talk to the licensure staff directly, please call (614) 466-3934 and follow the prompts to the Licensure Department.  It is better if the applicant calls directly, since some information cannot be released to other parties.
